WAIVER AND RELEASE OF LIABILITY

In consideration for being allowed to participate in the TSF x NBA Academy Scrimmage Game, I, the undersigned participant (or parent/legal guardian if under 18), acknowledge, understand, and agree to the following:

1. Assumption of Risk
   I understand that participating in any basketball activity involves physical exertion and potential risks of injury, including but not limited to sprains, fractures, contact injuries, or serious bodily harm. I voluntarily assume all such risks associated with my (or my child’s) participation in this scrimmage.
   

2. Release of Liability
   I hereby release, waive, discharge, and covenant not to sue The Skill Factory, NBA Academy, its owners, officers, directors, employees, volunteers, affiliates, sponsors, and facility owners from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, illness, or loss sustained during or as a result of participation in the scrimmage game.
   

3. Medical Treatment Consent
   I authorize The Skill Factory staff and volunteers to obtain emergency medical treatment for me (or my child) should the need arise. I understand I am responsible for any costs incurred as a result of such treatment.
   

4. Photographic Release
   I grant The Skill Factory and NBA Academy permission to capture and use photographs, video recordings, or other media of me (or my child) for promotional and marketing purposes without compensation.
   

Code of Conduct
I agree to abide by the rules and instructions of the event organizers and staff. I understand that failure to comply may result in immediate removal from the scrimmage with no refund.
